I thought his attempted clearance for Andy's second was very poor - I know he's short but he got good contact on it and was at least a foot in front of the line but he headed it straight into the top corner. Great finish.

....
Was it a goal kick though or a keeper clearance?
neither MOTD2 or the Sky highlights showed Adrain kicking it for some reason.

Pretty sure it was a goal kick, as from my seat in the Rio lower I noticed Carroll was waiting in an offside position and then reminded myself that it was a goal kick.

Collins had just come on and was defending their free kick - he went up with the Swansea player and put him off such that it was headed towards the corner flag for a goal kick. Swansea then made a substitution and they were showing the player that came on whilst Adrian took the goal kick.
